Vanguard Intake

Please complete the following Intake Survey and Vanguard Secondary Application as thoughtfully and as best as you can. After Vanguard has received this application, as well as the school-based team referral from your current school, you may expect a call within 2-3 weeks to arrange a Preliminary Interview. As a reminder, all information received is kept in agreement with the Freedom of Information and Protection of Privacy Act.

  • Enrollment into Vanguard is done Quarterly ( September, November, February and April).

    Support can include: programs, people who have directly supported you, technology tools or supportive materials that have been helpful.

  • Upon completion of this form, the school intake team will review your submission and reach out to the guardian in a timely manner. In addition to this form, we require that the most recently attended school also submit a referral before a preliminary interview with the student and famliy is scheduled.
